We offer a direct primary care service arrangement focused on on-demand care including, urgent care, physical examinations, and the diagnosis and treatment of many common illnesses, skin issues, minor injuries, infections, and everyday health needs — with certain in-house testing and X-ray support when clinically appropriate and available. If you are experiencing an emergency, call 911.

All covered services are subject to clinical judgment and may vary by location. Where clinically indicated, members may be referred to a higher level of care which is not covered under their membership. Telehealth availability varies by state.

Colds, flu symptoms, sore throat, cough, congestion, fever, nausea, vomiting, diarrhea, ear pain, sinus issues, and other common non-emergency illnesses
Skin issues
Minor/mild: rashes, bites, allergic reactions, burns, skin infections, abscesses, and wound concerns
Minor Injuries
Sprains, strains, minor cuts, bruises, back pain, joint pain, sports injuries, and possible minor fractures

Chest pain, stroke symptoms, severe shortness of breath, major trauma, heavy bleeding, severe allergic reaction, loss of consciousness, or any condition that may threaten life or limb. Call 911.

Complex chronic disease management, surgical care, advanced cardiac workups, advanced imaging, oncology, or conditions requiring hospital-level monitoring

Common questions

Does this replace our health insurance?
No. This is not insurance. It is a Membership Program that provides discounted access to a direct primary care service arrangement focused on on-demand care including, urgent care, physical examinations, and the diagnosis and treatment of many common conditions only. It works alongside your existing health plan — not instead of it. It can be particularly effective when paired with HDHPs.
Can we offer this to part-time and contract employees?
Yes. There is no minimum hours requirement. Part-time, seasonal, 1099, and uninsured employees are all eligible. Our goal is for you to be able to offer this to your entire workforce, regardless of employment status.
Is there a minimum number of employees to enroll?
No minimum. Two employees or two thousand — the Membership Program is available to any employer of any size. Employers with fewer than 5 enrolled employees may be subject to a small monthly administrative fee.
How does this work with our HDHP?
This Membership Program can be an effective complement to your HDHP. Your employees keep the lower-premium HDHP for major medical events and can use the Membership Program for the everyday care that they would otherwise have to pay for out of pocket until they meet their annual deductible. You keep your premium savings, employees get affordable access to care.
How do we get started?

Commitment
Annual, billed monthly. Auto-renews each year. Written notice of non-renewal required at least 70 days before your anniversary date.
Termination
Either party may terminate for convenience with 60 days written notice.
Billing
Single consolidated monthly invoice for all enrolled employees. No per-claim processing.
Small groups
Employers with fewer than 5 enrolled employees may be subject to a monthly administrative fee.
Billed separately
Outside lab testing, some procedures, non-covered services, DME, vaccinations, and immunizations are not included as membership benefits and billed at time of service or directly by the lab or vendor.
